The registered nurses and social workers in the department assist patients to achieve optimal clinical outcomes and appropriate safe post hospitalization transition plans. They monitor all patient care for quality, utilization and psychosocial needs and are available to assist with home care services, acute or sub-acute rehabilitation and nursing home placement.

Both departments can be contacted between the hours of   8:00 am and 5:00 pm, Monday through Friday. If you need further assistance, ask your practitioner or unit nurse to contact the appropriate department. The offices are located on the 4th floor of the South Annex.
