Dialysis services offer a comprehensive approach beginning in a hospital-based unit where social workers, nephrologists and skilled nurses assist the patient in choosing the correct modality. These may include in-center hemodialysis, peritoneal dialysis, a home modality, or transplantation. When a decision has been made and dialysis begins, the skilled staff is there to assist and prepare the patient for dialysis as an outpatient.

The hospital-based Unit, as well as the outpatient Centers in Bay Shore and Lindenhurst, contains state-of-the-art equipment. Each was designed to create an environment of optimum comfort during treatment, including individual televisions and specially designed reclining chairs.

From Southern State Parkway:

Southern State Parkway to exit 41 South (Bay Shore Road). Continue past Hess gas station. At Bay Shore Inn (purple building), make the first right turn. This brings you into the rear entrance of the parking lot.

From Sunrise Highway:

From east: get off at Fifth Avenue, exit 42, and onto the service road. Continue past Peter Pan Diner. We are on the right side of the road. From west: continue to Fifth Avenue exit and go over Sunrise Highway and back on to the service road going west.

Diabetes Care Center

The Diabetes Care Center of Good Samaritan is dedicated to providing excellence in outpatient diabetes education in recognition of the fact that a diagnosis of diabetes requires the patient to be trained in self-management skills.

The outpatient setting provides a forum in which to explore all aspects of diabetes self-management, including nutritional and pharmacological management, monitoring of blood sugar and the prevention and treatment of long and short-term complications. Each patient is carefully assessed by the diabetes nurse educator in order to tailor the program to their individual needs.

The Diabetes Care Center is an American Diabetes Association recognized program, meeting rigorous standards of education to ensure that the program is providing accurate and current information. The program consists of four classes, which meet once a week for two hours, and a follow-up class two months after completion of the program, for a total of 8-10 hours of education. All classes are taught by registered nurses and registered dietitians who are certified in diabetes education. Each patient is provided with a complete printed syllabus of the program and current adjunct educational information.

With the assistance of the educators, each patient will set a behavior change goal designed to improve their diabetes control. They will also be provided with an individual meal plan and have the opportunity to practice creating a menu based on their own plan. All of the medications used in the treatment of diabetes are discussed, with emphasis on mode of action and relationship to high and low blood sugars.

The program offers daytime and evening classes to accommodate the various schedules of our patients. The patient’s referring physician will receive written notice of the patient’s completion of the program, along with any pertinent information and recommendations.

Most insurance is accepted. The Center is located at Good Samaritan Hospital, 1000 Montauk Highway , West Islip.

The Martin Luther King, Jr. Community Health Center, located in Wyandanch, represents a cooperative effort between Good Samaritan Hospital Medical Center and the Suffolk County Department of Health Services. When the Health Center opened its doors on July 1, 1968, it was the first community health center in Suffolk County and a model for others that followed on Long Island.

Today it continues the tradition of providing much - needed healthcare to the surrounding community. The services include family practice, prenatal, pediatrics, immunizations, social work and diagnostic testing.

Good Samaritan Nursing Home

The Good Samaritan Nursing Home is located at 101 Elm Street in Sayville, one block from the Great South Bay. The 100-bed facility has been part of Good Samaritan since 1980. The residents of the Nursing Home are a medically complex population requiring the full-time care only a skilled nursing facility can provide.

From the moment of admission the inherent value and dignity of life of each resident is always our consideration. This skilled nursing facility emphasizes an interdisciplinary approach to meet the needs of each resident. This begins before the resident comes to live in our home. Early in the admission process, family and prospective residents meet with our team members representing all departments. We believe that seeds to foster confidence and relationships must be planted as soon as possible, since admitting a loved one to a nursing home is never easy.

Our 100-bed size allows for personal attention to individual needs. All team members are easily reached by residents and families. Even our head chef knows each and every resident and their specific nutritional needs and/or wants. Every effort is made by our capable dietary staff to provide customized entrees and nutritional snacks. We never lose sight of the social significance of sharing a meal with another person. A variety of food accompanies our excellent recreational programs.

Our physical location is uniquely beautiful. Good Samaritan Nursing Home is situated in a lovely residential area of Sayville within a one block walking distance of the Great South Bay. Our residents frequently enjoy a picnic lunch at the seashore during the summer months.

The spiritual needs of our residents are very important to us at Good Samaritan. A full-time Chaplain, priestly presence, and a lively core of pastoral care volunteers provide daily coverage to enhance a living faith for all denominations. This includes the prayerful and supportive presence of staff to residents and family members during times of crisis, particularly that of dying and death.

At Good Samaritan, we are fortunate to have the presence of the Good Shepherd Hospice Program available to residents who are terminally ill. This provides uniquely for family and palliative supports to continue in an institutional setting.

Long Term Home Health Care

Illness doesn't just affect the patient; it has an impact on everyone in the family. The caregiver in the household requires the peace of mind to know that the patient's needs are being attended to in the best possible way. But more than that, the caregiver also needs some time off to do the things with his or her family that provide for quality in the way they live. Good Samaritan's Long-Term Care services provide patient, caregiver and caregiver's family with the skill, support, counsel and respite time so necessary when illness strikes.

Good Samaritan's Long-Term Home Health Care Program (LTHHC) can bring new life to adults who would otherwise be eligible for nursing home placement. The mission of the program is to provide cost-effective services to chronically ill people who are unable to function independently but to not require around-the-clock nursing care.

Certain life circumstances could create an environment where individuals or families would benefit from the LTHHC program. Typical scenarios include a married couple with one spouse suffering from chronic health problems, an elderly mother with a cardiac condition caring for a mentally retarded adult child or an elderly adult with early dementia, who is socially isolated. These cases would be individually assessed for appropriate patient care. Many frail elderly adults living on their own could benefit from daycare services including nursing supervision, assistance with medications, personal care aides, social work supportive services, physical, speech, occupational, respiratory, and nutritional therapies. Both the patient and caregiver can receive support through the LTHHC program.

Durable medical equipment including bathroom safety equipment, wheelchairs and walkers are also available. LTHHC can provide Federally Waivered Service such as Personal Emergency Response Systems (emergency call buttons), housing improvements such as ramps and electric stair glides, moving assistance, respite services and Social Day Care.

The primary payer for this program is Medicaid, however, patients can access services as self-payers or with long-term care insurance coverage. Eligibility is based on meeting criteria for nursing home admission, and having complex needs requiring multiple services and case management.
